The subscriber must complete a Change of Status Form and attach a copy of the birth certificate. The subscriber has 30 days from date of birth to enroll newborn child to the plan. |

Yes! You have 30 days from the time school started to enroll your child. The Subscriber must complete a Change of Status Form to enroll child and your child must complete a Part II form. The completed forms and a copy of the full time school verification must be submitted to StayWell within 30 days from the date school started. |

My wife will be quitting her job with her employer and I would like to pick up the insurance for my family. What do I need to do? |

You must complete an Enrollment Application, attach a copy of the HIPAA Certificate from spouse’s employer or previous insurance, complete a Part II and submit all documents within 30 days from the time spouse lost coverage. |

I am currently enrolled under the Bronze Plan with the Government of Guam. Can I cancel my plan now or do I need to wait until Open Enrollment? |

The Bronze Plan has a locked in provision. The only time you can cancel is during Open Enrollment. |

You must complete a Change of Status to add dental. In order for you to add dental, you must have had medical for one year, and dental can only be added in December. |
